How would a photographer go about getting an image published in a magazine?

I am a photographer and sometimes I stray from portraits and weddings to do a bit of abstract, darker work. I have been visiting alot of photography blogs and alot of them have either been featured on other popular blogs, the online news or in magazines that I read myself. Like Inked, Gothic Beauty, YWD, and so on.

You contact the picture editor and arrange for a viewing of your book (portfolio).
